What are Questionnaires?

Questionnaires are a way to collect data from a client that is question and answer based (Q & A or decision-tree).

Let’s take a contrived example. Let’s say you need to know if clients have a middle name. If they do, then you want to know if their middle name is a family name. If they don’t, then take them down a different line of questioning.

Your line of questioning might look like this:

Q: Do you have a middle name?
A: Yes
Q: Is this a family name?
A: Yes
Q: Do all your siblings share this same middle name?
and so on and so forth…
A: No
move on…
A: No
move on…

A more real life example might be if you need to collect information for government forms and then your Q&A might start looking something like this:

 

Perhaps there is information you want to collect from new clients and each question is based on the previous answer to a question. This is what questionnaires are used for.
